Steps To Acces VA Home Health Care
Check Eligibility
- Call your local VA Medical Center or visit www.va.gov to confirm eligibility.
- Speak with Your VA Primary Care Provider (PCP)
- Discuss your needs with your PCP, who can submit a referral to VA home health care programs.
- Explore the VA Community Care Network
- If VA services aren’t locally available, the VA may connect you with non-VA home care agencies.
- Work with a Case Manager
- Veterans with complex needs can be assigned a case manager for ongoing support and coordination.

Private home care
Private home care refers to non-medical support for daily living and companionship.
